Each patient treated at EOSC has the right to:
- Receive the care necessary to help regain or maintain your maximum state of health, without regard to age, race, sex, national origin, religion, culture, physical handicap, personal values or belief systems.
- Be treated with dignity and respect, expect your personal privacy to be maintained, and have all communications and records be kept secure and confidential in compliance with all state and federal laws.
- Approve or refuse the release of medical records to any individual outside the facility, except in the case of a transfer to another health facility, or as required by law or third-party payment contract.
- Expect personnel to be friendly, considerate, respectful, and qualified through education and experience to perform the services for which they are responsible with the highest quality of service.
- Be given the name of your attending physician, the names of all other physicians directly assisting in your care, and the names and functions of other healthcare persons having direct contact with you.
- Be fully informed of the scope of services available at the facility, provisions for after-hours and emergency care and related fees for services rendered.
- Be a participant in decision regarding the intensity and scope of treatment. If you are unable to participate in those decisions, your designated representative or other legally designated person shall exercise your rights.
- Consent to the surgery being performed and anesthesia administered based on the benefits and risks identified by the surgeon, anesthesiologist and staff.
- Refuse treatment to the extent permitted by law and be informed of the medical consequences of such a refusal. Accept responsibility for your actions should you refuse treatment or not follow the instructions of the physician, anesthesiologist or facility staff.
- Be advised of participation in a medical care research program or donor program, give consent prior to participation in such a program, or refuse to continue in a program that you have previously given informed consent to participate in.
- Request that a copy of any Advance Directives or Durable Power of Attorney be placed in your medical chart for reference by another facility in the case of a transfer. Note that EOSC does not honor Advance Directives.
- Reports of pain will be believed and responded to quickly. Information will be given to you about pain and pain relief measures.
- Expedient and professional transfer to another facility when medically necessary and to have the responsible person and the facility that you are transferred to notified prior to transfer.
- Receive care in a safe setting and have the ability to express safety concerns, grievances or conflicts regarding your care at any time to a Clinical Manager or Administrator, without the threat of retribution, abuse or harassment. You are entitled to have your concern investigated by appropriate staff in an effort to provide a timely resolution.
Each patient treated at Elmhurst Outpatient Surgery Center has the responsibility to:
- Respect the property of others and the facility. Be considerate of other patients and personnel. Follow the rules and guidelines of the facility.
- Provide all caregivers, to the best of your knowledge, the most accurate and complete information regarding present complaints, past illnesses/hospitalizations/surgeries/anesthetics, medications, unexpected changes in your condition or any other medical information, which may have a direct effect on the provider at the Surgery Center.
- Accept consequences for outcomes related to not following prescribed care guidelines.
- Provide EOSC with all information regarding third party insurance coverage. Promptly fulfill all financial obligations to the facility, less any third party payment or contractual allowance.
- Inform EOSC if you have Advanced Directives or Durable Power of Attorney. Note that it is not the policy of EOSC to honor these; however, they will accompany your records if you are transferred to another facility.
- Follow pre-operative and post-operative instructions given by your surgeon, anesthesiologist or surgery center staff. Inform your caregiver at EOSC if you do not understand these instructions or if you have any questions or concerns about your ability to follow and comply with the proposed plan of treatment.
- Work cooperatively with the surgeon, anesthesiologist and nursing staff to assess pain and communicate if your pain is not relieved. Communicate any concerns regarding taking pain medications to your surgeon. Work with your surgeon to develop a pain management plan that is appropriate for your needs.
- Indicate if you feel your privacy is being violated or that your care is being threatened. Request to speak with a Manager or Administrator to share your concern and file a grievance so that your issue may be investigated and a timely resolution obtained.
