Medicare Patients:
EOSC does not participate in United Healthcare’s Secure Horizons product or Aetna Better Health. EOSC encourages all patients with Medicare replacement plans administered through a commercial carrier to contact the carrier to determine if EOSC is participating before scheduling surgery.

HMO Patients:
HMO products including Aetna HMO, Blue Advantage HMO, HMO Illinois and Humana HMO are affiliated with the Elmcare only.

Plans No Longer Accepted:
EOSC no longer accepts cases in litigation or US Department of Labor Worker’s Compensation.
